Here are the forms that are commonly used to order different types of telecommunications services. Should you have questions about ordering services, please email cnsdesign@vt.edu. More information about these forms and departmental billing is available.

• Interdepartmental Communications Request (ICR) > Use this form to order voice, data, and video service
• Request for Project Estimate > Use this form to request an estimate from CNS for a proposed project
• Banner Change Form > Use this form to change the banner account for monthly charges
• Virginia Tech Modem Pool (VTMP) Application > Use this form to order individual modem pool accounts
• Wireless Network > Information about the campus wireless network (802.11b)
• Departments May Sponsor Guests' Short-term Access to the University's Wireless Network > Use this form to set up sponsorship
• Off Campus Virginia Tech Proxy > Instructions for accessing Virginia Tech sites through third party ISPs
• Web Access Request Form > Request access to view and print monthly invoices for departmental telecommunication services
